Fill your cash prescription for Gonal–f®
through Freedom Fertility Pharmacy™ for two cycles of treatment. First
cycle must contain a minimum of 900 IU. Second cycle must contain a
minimum of 900 IU for a cash savings of $250 or a minimum of 2100 IU
for a cash savings of $500. Enroll in FertilityAssist 2 between
December 1, 2008 and June 30, 2009. Second prescription must be
received and shipped by Freedom Fertility Pharmacy™ on or before
September 30, 2009. Only one card may be redeemed at a time and cannot
be used in conjunction with any other offer or discount program.
